PreTRM Connect Terms & Conditions
Version 1.0
Effective Date: August 28, 2026
1. Acceptance of Terms
These Terms and Conditions (“Terms”) govern access to and use of the PreTRM Connect platform, website, questionnaires, communications, educational content, and related services (collectively, the “Service”) provided by Sera Prognostics, Inc. (“Sera,” “we,” “our,” or “us”). The Service may be accessed and used by a patient for whom PreTRM® testing was ordered or by a healthcare provider (“HCP”) who ordered such testing for such patient (in each case a “User”).
By accessing or using the Service, the User acknowledges that the User has read, understood, and agrees to be bound by the Terms in effect on the date of such access or use.
2. Description of the Service
PreTRM Connect is a digital communication and care-support platform designed to collect information from enrolled patient Users regarding their pregnancies and to communicate that information to authorized HCP Users. The Service may include:
-
Pregnancy-related questionnaires;
-
Symptom and medication adherence reporting;
-
Educational content;
-
Notifications and reminders;
-
Provider reports and summaries; and
-
Other related features and functionality.
The Service is intended to support communication between patient Users and their HCP Users and may generate reports or summaries based on information submitted by each.
3. Not Medical Treatment or Advice
Sera is not an HCP. Sera does not, through the Service or otherwise:
-
Diagnose any disease or medical condition;
-
Provide medical advice;
-
Recommend treatment;
-
Direct clinical care; or
-
Replace consultation with a qualified healthcare professional.
Each patient User and HCP User agrees and acknowledges that all healthcare decisions remain solely the responsibility of the treating HCP and patient. Patient Users should always seek the advice of their HCP regarding any medical condition or treatment decisions.
4. Emergency Situations
The Service is not intended for emergency use. If a patient User is experiencing a medical emergency, the User should contact emergency services immediately, call 911, or seek prompt medical attention. No User may rely on the Service, electronic messages, emails, or questionnaires for emergency communications.
5. Patient User Eligibility
Each patient User represents and warrants that:
-
The User is at least 18 years of age or has the capacity or authority to use the Service under applicable law;
-
The information the User provides is accurate and complete;
-
The User will use the Service only for lawful purposes; and
-
The User has been authorized, invited, or enrolled by an eligible HCP or healthcare organization.
6. HCP User Responsibilities
Each HCP User agrees to:
-
Provide truthful and accurate information;
-
Promptly update information that becomes inaccurate;
-
Protect the confidentiality of the User’s login credentials;
-
Notify Sera of any unauthorized access to the User’s account; and
-
Use the Service in accordance with these Terms and applicable law.
Each User is solely responsible for information submitted by such User through the Service. By submitting information through the Service, each HCP User represents and warrants that:
-
The User will only use the Service for patients who have tested positive (i.e., “higher risk”) with the PreTRM® test;
-
Any information submitted by the User about a patient is accurate; and
-
Any patient for whom the HCP User submits information has been made aware of the Service and has agreed to the submission and use of such patient’s information in the Service.
7. Communications
By participating in the Service, the patient User consents to receive communications relating to the User’s enrollment and use of the Service, including:
-
Notifications which can include things like email, telephone calls, SMS, etc.;
-
Questionnaire reminders;
-
Service announcements;
-
Administrative communications; and
-
Other communications reasonably related to the Service.
Certain communications may be delivered through third-party communication providers on Sera’s behalf.
In most cases, the Service involves patient Users directly responding to questionnaires (e.g., inputting their own information into the PreTRM® Connect platform). In some cases, an HCP User or Sera personnel may contact the patient User to collect the questionnaire information and use such information in the Service (e.g., input it into the PreTRM® Connect platform) on the patient User’s behalf. By providing such information to the HCP User or Sera personnel, the patient User:
-
Consents to such information being used in the Service, including being input into the PreTRM® Connect platform; and
-
Authorizes and directs the use and disclosure of such information as permitted by HIPAA and Section 9 of these Terms.
8. Educational Content
The Service may provide educational information regarding pregnancy, preterm birth, symptoms, medications, or related topics. The User agrees and acknowledges that educational content is provided for informational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment.

The User acknowledges that such notifications and communications may contain protected health information (“PHI”), as such term is used in the Health Insurance Portability and Accountability Act (“HIPAA”). The User hereby authorizes Sera to disclose such PHI to an HCP User and to any third-party as reasonably required for Sera to provide the Service. Use of a User’s personal information, including but not limited to PHI, is governed by the applicable Privacy Policy, which is incorporated into these Terms by reference.
10. Healthcare Provider Access
